To opt in to the School Readiness Program Assessment (SRPA), 
Please type or write your name and date below and return it to shollis@elcpinellas.org.   

I participate in the School Readiness Program Assessment and agree to follow the participation requirements outlined in the Florida Administrative Code, Rule 6M-4.740.  Review the rule HERE.  I understand I am exempt, but by participating, I will receive the benefits of SRPA, including School Readiness differential payments and incentive opportunities through the Early Learning Coalition.  I can opt out anytime by notifying us before May 1st of the current contract year.
